Community Fitness & Wellness Facility Overview In the effort to provide a continuum of care in the area of health the Community Fitness and Wellness Facility will provide individuals with disabilities the opportunity to be ‘fit for life’. The Community Fitness and Wellness Facility at Frazier Rehab Institute is an activity-based exercise program designed specifically for individuals with physical disabilities within the community. This fully accessible facility is designed to assist clients in the improvement of cardiovascular/aerobic fitness, muscular strengthening and flexibility. Membership Packages and Fees The enrollment fee is a one time payment of $100.00 for all packages which includes (2) Free Personal Trainer Sessions
*Sessions to be scheduled a minimum of 48 hours in advance Discounted rates for students and seniors are available for all packages. Hours of Operation Monday through Friday from 5 p.m. - 8 p.m. and Saturday from 10 a.m.- 3 p.m. Hours are subject to change and the facility will be closed for all major holidays. FREQUENTLY ASKED QUESTIONS What are the requirements for becoming a member of the Community Fitness and Wellness facility? Becoming a member of the Community Fitness and Wellness program is very easy. The only requirement is that you must be an individual with a permanent physical disability. We do allow family members and friends to join for an additional fee, but they are required to be sponsored by an individual with a physical disability. How do you sign-up to become a member? It is required that all members complete a membership application prior to coming to the facility. The application can be obtained by clicking here (Community Fitness and Wellness Application). Once the application is complete and received, you will need to schedule an appointment by calling (502) 582-7411 or (502) 409-0035 to meet with a member of the Community Fitness team to go over the details of the application and to participate in new member orientation. Once these steps have been completed you will be an active member of the gym. Where is the Community Fitness and Wellness Facility and when is it open? The Community Fitness facility is located at Frazier Rehab Institute, 220 Abraham Flexner Way, Louisville, Kentucky on the sixth floor in the outpatient gym. The facility is open Monday thru Friday from 5 to 8 p.m. The facility will begin weekend hours based on the needs of the members. Weekend hours will range from 10 a.m. to 5 p.m.. Facility hours are subject to change and will be communicated to all members in advance. In case of inclement weather please call (502) 582-7411 or (502) 409-0035 for cancellations. How long do I have to become a member? All members are asked to sign a three-month membership agreement. If you decide to cancel after three months, we ask that you submit a written notice of your intention to cancel. This must be delivered or mailed prior to the first of the month and you must bring your account balance to zero. What kind of equipment do you have at the facility? This is a fully accessible facility designed to improve cardiovascular/aerobic fitness, muscular strength and flexibility through the use of specialized equipment for individuals with physical disabilities. How much does it cost to be a member of the Community Fitness and Wellness Facility? We offer a variety of packages and prices to our members based on their fitness needs and goals. Here is a summary of what each of the packages includes and cost. Basic Gym Membership: This package includes a general orientation with a member of the fitness team and use of all basic gym equipment during all hours of operation. This package does not include use of a trainer or participation in Locomotor/FES training. You may upgrade your membership and purchase a trainer and/or Locomotor /FES package at any time during your membership.
Package A- Basic Membership + Locomotor/FES Training: This membership includes use of all gym equipment, plus 3 weekly sessions of Locomotor training (12 sessions per month) AND a minimum of 2 weekly sessions of FES cycling (8 sessions per month). $1050.00/month**(All Ages) Package B- Basic Membership + Locomotor/FES Training: This membership includes use of all gym equipment, plus 2 weekly sessions of LT (8 sessions per month) AND a minimum of 3 weekly sessions of FES cycling (12 sessions per month). $850/month*** (All Ages) Package C- Basic Membership + FES: This membership includes use of all gym equipment, plus unlimited sessions on the FES bike. $650.00/month** (All Ages) When are Locomotor and FES sessions scheduled? All sessions for Locomotor training and FES cycling must be scheduled 48 hours in advance unless the equipment is open for use. All sessions are scheduled in one hour increments. A member is required to vacate open equipment if a scheduled member arrives. If a session has to be cancelled, you are expected to notify a member of the team within a minimum of six-hours prior to the scheduled session. What is a Trainer Package? A trainer package can be purchased during any time, but all trainer sessions must be scheduled 48 hours in advance. A trainer package includes a one-hour session with an exercise specialist who will develop a specific exercise plan for the member to follow to assist them in meeting their fitness goals. The trainer may also assist in teaching the member appropriate exercise techniques and positioning in order to achieve the best results. The trainer can also provide consultation for exercise routines to follow on a weekly and/or monthly basis. Single Personal Training Session - $40/hour. Personal Trainer Packages
Who should purchase a trainer package? We will recommend for you to work with a trainer if you do not have experience in participating in regular exercise and if you require assistance when performing fitness activities. We want everyone to get the most out of their session at the gym and will make recommendations to each member upon joining the facility based on the outcomes of your individual fitness assessment. We do provide basic set-up assistance on equipment and a general exercise guide for all members to follow. If you have specific fitness goals that you want to achieve, such as targeting specific muscles groups, we recommend at least purchasing one session with a trainer to help you create a plan to begin working towards these goals. |
