Stroke Rehab Program

Frazier's Stroke Rehab Program emphasizes teamwork so that patients can return to their homes and communities at their highest functional levels.

An interdisciplinary team of medical and clinical professionals help the patient throughout the rehab process. The Frazier team sees the patient and family as key participants in therapy, personal care, stroke education and planning the return home.

Stroke patients enter Frazier at different levels of recovery. The rehab team expects variations in abilities and extent of recovery, therefore, the rehab stay at Frazier is tailored individually. Following evaluation, the rehab team determines each person's needs to maximize recovery.  Available Services include individual and group therapy, rehab nursing care, routine medical care and consultation, specialized diagnostic testing and family teaching.

Family education helps families prepare for the patients' return home. Group therapy addresses daily living skills, communication, self-feeding and swallowing needs.

The stroke team focuses on improving patients' communication abilities and physical and/or cognitive capabilities - all essential to mobility, self-care and psychosocial adjustment to stroke.

Prior to inpatient discharge, follow-up meetings that may be needed are arranged. Frequently, patients return to one of Frazier's outpatient facilities for therapy. Specific patient needs are addressed by the outpatient rehab team, with the purpose of reaching each patients' full potential. Frazier also offers a stroke support group with monthly meetings for both stroke survivors and family members, and weekly family meetings.

Patient and Family Stroke Education Overview

The experience of having a stroke can be overwhelming. The emergency room, the medical procedures, and just being in the hospital can be traumatic. The unknowns are frightening. “Will I feel like this forever?” “Will I walk again?” “Will I talk again?” “Will I be able to care for myself again?” “Will I have another stroke?”. The list of questions and confusions can seem endless.

We have put this handbook together to give you and your family answers to the questions you already have, and even some you haven’t thought of yet. For many stroke survivors, the road to recovery may take quite a long while and you will have to navigate through various health care systems. This educational material can guide you through your stroke recovery while you are with us at Jewish Hospital and Frazier Rehab Institute and beyond.
